  • *Please take a moment to fully REVIEW our office policies stated below prior to your future appointments as there are NO exceptions to ANY of our practice rules, office policies, safety/health regulations, and privacy protocols, whatsoever.
  • *Payment Policy : All sessions must be prepaid in advance of weekly appointments and are NON-REFUNDABLE. We process ALL credit card payments for ALL future professional services and financial transactions in advance of your weekly professional meetings and/or telephone appointments. Our professional rates applies to ALL telephone contacts or any in-person meeting with you or anyone on your behalf.
  • *We are unable to accept personal checks and do NOT accept payments from any third-party payer to safeguard your privacy and confidentiality.
  • *Refund Policy : Please note, there are absolutely NO refunds for ANY of your prepaid sessions in case of broken appointments by you as the professional times and days were reserved for you and considered as broken appointments. Therefore, we ask that you do NOT make appointments with ANY of the doctors unless you fully intend to keep all your future weekly appointments as we do NOT issue refunds on ANY prepaid appointment. You may notify us one WEEK prior to your appointment day (excluding weekends, holidays, and weekdays before NOON), if you wish to change appointment time ONLY SUBJECT TO STRICT AVAILABILITY OF YOUR DOCTOR as our practice is fully committed based on previously scheduled times 2 weeks in advance.
  • *Methods of contact : You may ONLY contact our office at 310-999-4996 for ALL questions, inquiries, and appointment requests. In strict compliance with the Federal HIPPA laws, we do NOT accept unencrypted text messages or any email communication, whatsoever. This is particularly important to safeguard patient confidentiality and privacy. Any repeated attempts to reach us via unencrypted email or text messages shall be grounds for termination of all professional services.
  • *Please note, we are NOT immediately available by telephone due to the majority of our staff working remotely during the COVID-19 global pandemic. Accordingly, ALL telephone calls will be returned within 24 HOURS excluding weekends, holidays, or weekdays after 1 pm.
  • *We require that all our patients provide our office with at least one valid telephone number. This is a requirement as our office may need to reach you for last-minute schedule changes as in case of emergencies presented to our office or the need to make potential changes to your appointments as a result of clinical emergencies.
  • *Please notify our office immediately if you are difficult to reach by telephone as we do require to have reasonable access to you by telephone (excluding email or text messages). This is a mandate of our practice in order to provide continuity of care to all of our patients and is a requirement during the course of treatment for trauma, depression, anxiety, personality disorders, mood disorders, and addiction recovery. Failure to respond to our telephone contacts or our inability to reach you by telephone shall be grounds for termination of all professional services particularly as service providers for addiction recovery and treatment of mood disorders.
  • We strive to fully observe state guidelines and be in compliance with the HIPPA Federal Law and requirements of ethical practices in providing psychological treatment for mood disorders, personality disorders, and addiction recovery.
  • Accordingly, we may need to suspend all future professional services, should we experience repeated broken appointments by you and/or difficulties in reaching you by telephone as in case of emergencies, disruption in continuity of your care as a result of missed appointments, or broken appointments by you.
  • *Termination of all professional services policy : Broken appointments, disruptions in continuity of your care as a result of missed appointments, or failure to present our practice with reasonable telephone access to you, may result in suspending all future appointments. Should we deem it necessary to suspend your future treatment, we will provide you with referrals to other community resources, so you may continue treatment for addiction recovery, depression, anxiety, trauma, personality disorders, and other mood disorders elsewhere.
  • *Emergency contact : In case of a medical or psychiatric emergency, please dial 911 or go to the nearest Emergency Room. If you are having thoughts of suicide or self-harm, please contact the Suicide Prevention Hotline at 1-800-273-8255. Please note, we do not provide emergency care services on-site and are unable to accept walk-ins at our offices.
  • *Confidentiality practices : Due to confidentiality, privacy reasons, and in accordance with the HIPPA Federal Law, we ask that you DO NOT CONTACT US VIA TEXT MESSAGE OR EMAIL. We are prohibited to communicate via unencrypted email or text message with any of our patients, whatsoever as we are unable to safeguard your confidentiality via text or email communication. We highly caution you not to breach your confidentiality by engaging in public telephone communication or unencrypted text communication with our office.
  • *OFFICE POLICY : Please remember to have your valid ID Card and Credit Card available at the time of your visit. Please be advised, a copy of your ID Card and the Credit Card shall be retained on file for identity verification and to secure advance weekly payments for ongoing professional services.
  • *Health, Safety, Confidentiality, and Privacy Mandates: We kindly ask all clients to observe Social Distancing and wear protective face masks when visiting our facilities, during all professional meetings, and at all times while in our building.
  • *Additionally, in an effort to observe client’s safety and confidentiality, we are NOT able to accommodate ANY unauthorized, unannounced or non-scheduled visitors in our building or in our offices due to CDC guidelines, health concerns, Federal HIPPA confidentiality mandates, and patient privacy protocols.
  • *IMPORTANT OFFICE POLICY DURING COVID-19 GLOBAL PANDEMIC:
    In accordance with strict State, County, and health guidelines particularly during the COVID-19 to intently safeguard the health and safety of our doctors, staff, and other patients, we reserve the right to suspend all in-person meetings with you, if you refuse to comply with the CDC, State and City guidelines, continue to engage in social gatherings with other households or continue to travel during the COVID-19 global pandemic. Should this occur, we will provide you with the option to complete all your pre-paid treatment sessions via telephone for at least 3 weeks. In case of disruptions in your weekly treatment, we will NO longer block weekly appointments for you and may opt to provide you with other referrals in the community to continue your future treatment.
  • *Important Office Policies and Note About Flu, Cold and Contagious Illness : (This policy is our standard office procedure at all times and without any exception. We want to take this opportunity to re-emphasize this policy in the wake of COVID-19).
  • *In the event that either you or your doctor is sick with a cold, flu, or contagious illness, or might be contagious, the appointment will *not* be canceled but you and your doctor will arrange to meet via telephone at your scheduled appointment time or another time per availability of your doctor in the week if needed.
  • *If you arrive at an appointment with a cold, flu, or contagious illness, your appointment will be rescheduled to a new time per our availability but you will be charged for the session as that time was reserved for you.
  • *Accordingly, we ask that you please notify us immediately and at least 24 hours in advance of your in-person appointment, if you are feeling sick or have a contagious illness so we may arrange for a secure telephone session at your previously scheduled time.
  • Please do NOT arrive early for any of your appointments to AVOID encountering other patients as we are not utilizing any waiting area in our building at this time.
  • *There is ONLY one person allowed in our building and/or office ONLY at the exact scheduled time with the doctor. Accordingly, there is no waiting allowed by you or anyone in our building area or in our offices at any time.
